Me neither. And it's consistent as well, I've tried formatting on Windows XP and Windows 7, as well as on 2 different versions of Linux. Oh, 3 different USB sticks as well.
I'll try a syslinux 4, just for the heck of it.

Nope, same result with syslinux 4.04. I tested making a boot usb stick (option 3 in your script), and it boots nice and clean, so the stick and computer cannot be the problem.
I'll boot up a 3:rd computer in Linux to test if that works better, it'll be a clean openSUSE 11.4 box.

Bite me... Works clean as a whistle in both creation and in booting when made on openSUSE 11.4.. The only catch is that it's very tricky to build apt-get, so I just symlinked to zypper, but that won't work in downloading mode. So, missing packages has to be downloaded manually. To sum it up: Tested NOT working on openSUSE 11.1 on multi-cpu x86_64..
