I've successfully set up a system with a Intel DQ45CB motherboard and a Intel E7200 cpu (no support for VT). I have a PCI card with USB EHCI controller that's been VT-d assigned to my Mythbuntu guest, no problems so far.
One thing that really bugs me is that the onboard USB-controller IHC10DO is NOT available for VT-d ;-(
I'm booting from a SATA disk in AHCI-mode so that shouldn't be my showstopper.
My theory goes towards that the motherboard lacks legacy keyboard/mouse support, only USB is available. I think that due to this the VT-d for onboard USB devices are disabled by the BIOS.
If anyone successfully managed to get the onboard USB available for VT-d please let us know!

Once you upgrade to ESXi 4.1 you should be able to passthrough USB devices directly with VMDirectPath.
