You'll find it in this file - download/file.php?id=200. Just extract the tgz and browse into it to find the driver.

Thanks heaps guys, it works sweet.
As you guys said in the above docs that to disconnect all devices from the SCSI card before loading the driver seems a good practice. I didn't do that at first so got a problem - the driver loading process was just hanging . . . Then I disconnect my legacy YAMAHA SCSI CD-ROM, everything's fine. I got my SCSI HDD and tape drive recognized, Awesome!

The simple.map and driver are being deleted after I reboot (ESXi on USB disk). How should I make it permanent?

Have you placed the files in an oem.tgz file - http://www.vm-help.com/esx40i/customize_oem_tgz.php

This solution doesn't works on ESXi v5.0.0 :( - any solution?

You would need a driver specific to ESXi 5
